Interesting. I've just tested it and it is working for me.

$ rspamadm configdump milter_headers
*** Section milter_headers ***
skip_authenticated = false;
use [
     "spam-header",
]
skip_local = false;
routines {
     spam-header {
         header = "X-Spam-Flag";
         value = "Yes";
     }
}

*** End of section milter_headers ***
