I am suspecting when the email comes in, it goes through rspamd because 
of milter, then goes to amavis then to clamav, then comes back to 
postfix and goes through rspamd again.

My only solution is to get rspamd to send the email to clamav. So we do 
not use amavis.

Please wait for a better answer because I am not an expert.
